Provide administrative and clerical support including answering calls and emails, scheduling and maintaining calendars, preparing documents, maintaining files and databases, ordering supplies, greeting visitors, coordinating travel and meetings, assisting with basic bookkeeping, and supporting team members.

Build and maintain relationships with physicians, case managers, discharge planners, SNFs, hospitals, and community organizations to drive referrals and census growth. Perform regular outreach (calls and visits), conduct client/family intake, document outreach and referrals, represent the organization at events, collaborate with internal teams, and gather feedback to improve service delivery.
