The role requires organization, accurate data entry, communication, customer service, confidentiality, and adherence to infection-control and safety procedures.

Develops hospital, physician, and community referral relationships to increase census and qualified admissions. Conducts outreach, educates referral partners, tracks referral activity, coordinates hospital-to-facility transitions, supports patients and families, and collaborates with admissions, clinical, and administrative teams to ensure smooth admissions and positive experiences.
