Coordinates referral networks, supports nurse home visitors, markets the Family Connects program, facilitates advisory board and quality-improvement activities, maintains resource directories and program records, and submits required reports. The role requires extensive stakeholder communication, knowledge of county health and social services, community outreach, and local and statewide travel.
