Lead client implementations of Carefeed’s Referral Management module for skilled nursing and post-acute care organizations. Assess referral, authorization, intake, documentation, and concurrent-review workflows; configure solutions; train cross-functional client teams; manage milestones through go-live; develop implementation resources; and partner with Product, Client Experience, and Support.
