The Aging Well Case Manager works collaboratively with various departments to provide case management services to individuals with developmental and intellectual disabilities. Responsibilities include conducting in-home visits, developing person-centered plans, managing documentation, and coordinating care with various stakeholders to address participants' needs.

The Senior Accountant will assist the VP of Finance with grant reporting, manage cash functions, oversee billing processes, ensure compliance with contracts, and prepare financial reports while enhancing financial functions and processes.
