PRN Licensed Practical Nurse for a long-term structured residence serving individuals with mental illness. Administer medications, monitor vital signs, assess physical health, and educate residents on healthy living and medication use. Philadelphia-based, LPN license required; experience preferred.

Join an ACT multidisciplinary team to provide vocational rehabilitation and supportive employment services for members with severe and persistent mental illness. Conduct vocational assessments, help members reach employment/education goals, connect them with resources and benefits, provide crisis support and education, and assist with maintaining community-based supports.

Provide recovery-focused, peer-delivered support for individuals with mental health and/or substance use challenges in a clinic setting. Use lived experience to build trust, advocate, document in EHR, collaborate with clinical teams, maintain boundaries, and participate in certification and continued learning.

Link clients to services, develop individualized service plans, advocate, navigate education/social/housing systems, assist with basic living needs, and support on-call crisis intervention. Majority of work is in-home or community settings with some office time.
