Provides in-home case management for children, youth, and families in foster care and transition programs. Assesses needs and risks, develops supportive relationships, coordinates treatment and educational services, advocates for families, monitors placements and progress, maintains regulatory documentation, and provides transportation. Collaborates with agencies, providers, foster families, and internal teams while ensuring compliance with program and licensing standards.

Coordinates and monitors individualized shared-living placements, supporting individuals’ health, safety, personal goals, and community participation. Provides case management, skill training, counseling, crisis intervention, home visits, service planning, caregiver support, and documentation. Collaborates with families, healthcare providers, caregivers, and community partners while maintaining regulatory, funding, and agency compliance.

Provides direct support to individuals with developmental disabilities through individualized and group activities, daily living assistance, transportation, transfers, and behavior-support strategies. Documents participant progress and behaviors, maintains a safe environment, follows individualized plans and emergency protocols, and collaborates with multidisciplinary staff. Helps participants build communication, social, sensory-motor, behavioral, and independent-living skills while accessing community resources.

Provides family-centered early intervention services for infants and toddlers, including developmental assessments, IFSP goal implementation, caregiver coaching, and home, community, and group-based support. Collaborates with multidisciplinary teams, maintains compliant documentation, participates in supervision and professional development, and travels extensively to service locations. Requires flexibility for occasional evenings and strong communication, organization, and interpersonal skills.

Foster parents provide a safe, nurturing home for children or teens in need of stability and support. Responsibilities include daily caregiving, transportation, emotional and behavioral support, appointments, documentation, and collaboration with case managers and the foster care team. KDC provides training, licensing assistance, 24/7 support, clinical guidance, respite options, and a tax-free stipend based on the child’s needs.

Conducts developmental assessments, develops and implements IFSP goals, coaches caregivers, delivers home and community-based services, collaborates with multidisciplinary teams, maintains documentation, and participates in supervision and professional development. Requires Portuguese bilingual proficiency, flexible scheduling, extensive travel, and direct work with children and families.

Lead and manage the Day Habilitation program, overseeing operations, staff supervision, regulatory compliance, person-centered service delivery, budgeting, community partnerships, and quality improvement to support adults with developmental disabilities.
