The Shared Living Case Manager coordinates and monitors individualized placements, ensuring the health, safety, and personal goals of individuals served. Responsibilities include case management, skill training, supportive counseling, crisis intervention, home visits, service planning, caregiver support, and accurate documentation.

The Case Manager collaborates with individuals, families, caregivers, healthcare providers, and community partners while ensuring compliance with state regulations, funding requirements, and agency standards.

Schedule: Monday–Friday, 8:00 AM–4:00 PM
Mileage reimbursement and regional differentials offered.

Qualifications

Required Qualifications:

  • Minimum of 2 years’ experience in case management, direct support, or a related human services role.
  • Knowledge of person-centered planning, developmental disabilities, and community resources.
  • Strong organizational, documentation, and communication skills.
  • Ability to work independently while maintaining collaborative relationships with teams and caregivers, and community partners.
  • CPR/First Aid Certified Trainer (or ability to obtain certification within designated timeframe).
  • Valid driver’s license, reliable transportation, and willingness to travel extensively.
 
 

Please note: Both individual physical support needs and caseload demands may fluctuate based on the individuals served, program requirements, and changing environmental or situational factors.

Kennedy-Donovan Center supports people with developmental delays, disabilities or family challenges to pursue their personal potential and success in the community. We fulfill our mission through a number of programs, which we offer in several regions: Early Intervention (EI), Early Intervention Partnerships, Healthy Families, KDC School (a 240-day developmental day school), Day Habilitation Services, Family Support Services/ Respite Care, Intensive Foster Care, and Transitional Job Support, as well as various residential support programs: Shared Living, Adult Family Care, Supported and Supervised Living, and Individual Support. Each year, our diverse and dedicated staff of 600+ employees and 300 respite home care providers and foster parents, serves over 10,000 children, adults and families with (or at risk for) developmental disabilities or delays, and their families.
