Provides direct, community- and home-based case management for individuals with developmental disabilities and their families. Responsibilities include assessing needs, developing individualized support plans, coordinating DDS, education, and community services, facilitating family supports, supervising respite providers, documenting services, attending ISP and IEP meetings, and supporting independence and community inclusion. The role requires extensive local travel, strong communication and organization, and compliance with agency and DDS standards.

Provides early intervention services to infants and toddlers and their caregivers. Responsibilities include developmental assessments, IFSP planning, home and community-based services, caregiver coaching, multidisciplinary collaboration, documentation, and professional development. The role requires flexible scheduling, extensive travel, reliable transportation, and compliance with early intervention standards.

Provide family-centered early intervention services to infants and toddlers birth to age three. Conduct developmental assessments, develop and implement IFSP goals, coach caregivers, collaborate with multidisciplinary teams, document services, and maintain compliance with early intervention standards. The role is community-based, involving travel to homes and community settings, flexible scheduling, occasional evenings, and use of documentation technology.
