Provide individualized case management, skill training, supportive counseling, crisis intervention, and community integration for adults with developmental disabilities. Conduct home and community visits, develop and review Individual Support Plans, coordinate community resources and healthcare, maintain documentation, provide transportation as needed, and collaborate with stakeholders to promote independence and well-being.

Provide home- and community-based case management for adults with developmental disabilities: coordinate placements, develop and review Individual Care/Support Plans, liaise with families and providers, maintain EHR documentation (iCentrix), ensure compliance with MassHealth, provide field coaching and crisis support, and participate in outreach and trainings.

Provide a safe, structured home for a child or teen, support daily routines and emotional needs, attend appointments, maintain documentation, and collaborate with KDC case managers and clinical staff.
