The Family Support Center Case Managers provide direct, community- and home-based supports to individuals with developmental disabilities and their families. This role facilitates access to services, builds natural and community supports, and promotes independence and community inclusion. The Case Manager acts as a primary contact for assigned families, collaborates with internal and external providers, and ensures adherence to agency policies, DDS regulations, and quality service standards.
Schedule: Monday - Friday; Flexible schedule to meet with families
Compensation: $20.99 - $23.49
Location: Plymouth County
Key Responsibilities:
- Serve as the primary contact for designated families and individuals, building supportive and trusting relationships.
- Assess family and individual strengths, needs, and preferences to develop Individualized Support Plans.
- Facilitate connections to generic and community-based resources to increase natural supports.
- Provide guidance, education, and skills training to families and individuals to promote independence, self-expression, and community integration.
- Support families in identifying and accessing appropriate services and resources, including generic community services.
- Coordinate with DDS, local education authorities (LEAs), and other community providers to ensure continuity of care.
- Attend and actively participate in ISP and IEP meetings as appropriate.
- Supervise and provide guidance to respite care and support providers.
- Maintain accurate and timely documentation, including service notes, progress reports, and required compliance records.
- Assist in outreach efforts, including development of marketing materials, community presentations, and informational forums for families.
- Participate in staff, clinical team meetings, supervision, and case reviews.
- Maintain compliance with all KDC policies, DDS regulations, and quality service standards.
- Perform other job-related responsibilities as assigned.

QualificationsRequired Qualifications:
- Minimum of 2 years’ experience in case management, direct support, or a related human services role.
- Commitment to community-based, family-centered supports for individuals with developmental disabilities.
- Competence in Microsoft Office and ability to use electronic documentation systems.
- Strong organizational, documentation, and communication skills.
- CPR/First Aid Certified (or ability to obtain certification within designated timeframe).
- Valid driver’s license, reliable transportation, and willingness to travel extensively
This position is community-based and requires travel to homes and community settings. Candidates must be comfortable working in a variety of environments and utilizing technology for documentation and communication.

Kennedy-Donovan Center, Inc. (KDC) is a 501(c)(3) non-profit organization founded in 1969 on the principle that all individuals have the right to participate and thrive in their community. Kennedy-Donovan Center supports people with developmental delays, disabilities or family challenges to pursue their personal potential and success in the community. We fulfill our mission through a number of programs, which we offer in several regions: Early Intervention (EI), Early Intervention Partnerships, Healthy Families, KDC School (a 240-day developmental day school), Day Habilitation Services, Family Support Services/ Respite Care, Intensive Foster Care, and Transitional Job Support, as well as various residential support programs: Shared Living, Adult Family Care, Supported and Supervised Living, and Individual Support. Each year, our diverse and dedicated staff of 600+ employees and 300 respite home care providers and foster parents, serves over 10,000 children, adults and families with (or at risk for) developmental disabilities or delays, and their families. We meet the needs of disabled individuals and their families throughout their life – from pregnancy, infancy, and childhood, into adulthood and the senior years.
