Why is this Role Essential?
Concerto is seeking an experienced healthcare partnerships leader to strengthen and scale how we engage with our partners and drive long-term growth across our markets.
The VP of Partnerships will lead the development and execution of Concerto's partnership strategy, with responsibility for building a high-performing relationship management function that strengthens existing partnerships, identifies growth opportunities, and contributes to increased patient census.
The ideal candidate has deep experience within skilled nursing, post-acute care, or an adjacent healthcare services environment and has previously built or scaled successful partnerships, business development, or client relationship function. This is an opportunity for an experienced leader to bring a proven playbook and shape how Concerto approaches partnerships at scale.
What Will You Do?
Partnership Strategy & Growth

Develop and execute a comprehensive partnership strategy designed to strengthen relationships and drive census and revenue growth.

Establish account segmentation and relationship strategies that prioritize Concerto's highest-value and highest-opportunity partnerships.

Identify opportunities to deepen partnerships, expand services, remove barriers to growth, and increase patient census.

Build and maintain relationships with key decision-makers across skilled nursing, post-acute care, hospitals, physician groups, nephrology practices, and other relevant healthcare organizations.

Leverage existing industry relationships while developing new strategic growth opportunities.
Team Leadership & Development

Build, lead, and develop a high-performing team of client relationship professionals.

Establish clear expectations, performance standards, operating rhythms, and accountability across the partnerships function.

Create a repeatable partnership management playbook that enables the team to assess relationship health, identify opportunities, resolve challenges, and drive growth.

Coach the team to operate as strategic relationship managers rather than solely responding to day-to-day partner needs.
Client Relationship Management

Establish a proactive relationship management model that positions Concerto as a trusted and strategic partner.

Implement consistent business review and executive engagement processes, including QBRs and other structured partner touchpoints.

Ensure visibility into partnership health, emerging risks, unresolved issues, and growth opportunities.

Translate partner feedback and insights into actionable opportunities across the organization.
Cross-Functional Leadership

Partner closely with operational leadership to ensure clinical, regulatory, staffing, service, and other issues affecting partner relationships are identified and addressed.

Serve as an effective connector between external partners and internal stakeholders.

Influence across functions and levels of the organization to remove barriers that impact partner satisfaction, retention, or growth.
Performance & Infrastructure

Define KPIs and leading indicators to evaluate partnership health, team effectiveness, and growth opportunities.

Establish disciplined CRM practices and ensure relationship activity, opportunities, risks, and outcomes are appropriately tracked.

Use market, account, and performance data to prioritize resources and identify opportunities.

What’s Required?

Significant leadership experience in partnerships, business development, account management, client relations, or a related function within healthcare.

Skilled nursing facility (SNF) or post-acute care experience strongly preferred.

Candidates from adjacent healthcare services—including home health, hospice, physician/MSO, renal, or other healthcare organizations serving or partnering closely with SNFs—will also be considered.

Demonstrated success building, scaling, or transforming a partnerships, business development, or client relationship function.

Proven ability to lead and develop high-performing teams while establishing clear expectations and accountability.

Track record of translating client relationships and business development activity into measurable census, referral, revenue, or market growth.

Strong understanding of healthcare relationship dynamics and the ability to engage effectively with executives, operators, clinicians, physicians, and other key stakeholders.

Established network within skilled nursing, post-acute care, or related healthcare sectors strongly preferred.

Strong strategic, analytical, communication, relationship-building, and influencing skills.

Ability to operate effectively in a dynamic, growth-oriented environment and build structure where processes are still evolving.
Reports to: EVP, Growth & Strategy
Salary Range: $175,000 - $225,000
