Nurse Case Manager (LVN) w/ Skilled Nursing Facility Experience

Arbor Hills, a Generations Healthcare is in search of a Nurse Case Manager to join our team.
The primary purpose of the Nurse Case Manager is to develop, evaluate, implement health care plans for individual patients, manage cost effective medical plans, while providing the highest level of patient care. The Case Manager also communicates treatment plans between families, doctors, providers and patients.
Essential Duties (include but are not limited to):
•Evaluate and update patient health care plans
•Assessment for new patients
•Review test results from medical exams
•Implement Physician orders and follow up with treatment plans
•Communicate with families, health providers and patients
•Documentation of medical records
•Provide emotional support to families and patients
•Provide education about health care plans to patients and families
Qualifications:
•Must possess, as a minimum, a Nursing Degree from an accredited college or university.
•Must have, as a minimum, one (1) years of experience as a supervisor in a hospital, nursing care facility, or other related health care facility.
•Must have training in rehabilitative and restorative nursing practices.
•Strong communication, excellent customer service, teamwork and organizational skills
•Must possess a current, unencumbered, active license to practice as an RN in this state.
•Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
•Be a supportive team member, contribute to and be an example of team work and team concept.
•Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
•Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities.

Skills Required
- Nursing degree from an accredited college or university
- At least one year of experience as a supervisor in a hospital, nursing care facility, or related healthcare facility
- Training in rehabilitative and restorative nursing practices
- Current, unencumbered, active RN license in the state
- Knowledge of nursing and medical practices and procedures
- Knowledge of laws, regulations, and guidelines applicable to nursing care facilities
- Ability to provide care appropriate to residents' age-related needs
- Strong communication, customer service, teamwork, and organizational skills
- Ability to deal tactfully with personnel, residents, families, visitors, government agencies, and the public
