What You Should Know About the LPN Extended Hours Role:

  • This is a Full-Time DRIVING position serving New Philadelphia and the surrounding communities.
  • The schedule consists of 4p-12a shifts one to three days per week, along with 8a-8p every other weekend. The position also includes rotating holiday coverage to ensure our patients receive consistent care when needed.
  • We are committed to delivering compassionate, dependable care and support to patients and their families during the hospice journey. This role is ideal for an LPN who is dedicated to providing comfort, dignity, and personalized care to those we serve.

The LPN Essential Duties Are:

  • The LPN will deliver skilled nursing services to patients in their homes and/or care facilities while following the established plan of care and working under the guidance and supervision of an RN. Responsibilities include addressing patient needs, supporting family members, communicating changes in condition, and maintaining accurate documentation of all care provided.
  • The LPN will serve as an important member of the hospice care team and help ensure each patient receives respectful, individualized care throughout their end-of-life experience.

LPN  Qualifications:

  • Current LPN license in the State of Ohio and must remain in good standing with no active board restrictions.
  • Previous nursing experience of one year or more is preferred.
  • Current CPR certification required.
  • Basic computer proficiency necessary for timely and accurate clinical documentation.
  • Must be comfortable driving to patient locations during daytime and evening hours, as well as weekends, holidays, and periods of inclement weather.
  • Must possess a valid driver's license and maintain a satisfactory driving history.
  • Must carry automobile insurance meeting applicable state minimum requirements.
  • Successful completion of a criminal background screening is required.
  • Successful completion of a 10-panel drug screening is required.

Skills Required

  • Current Licensed Practical Nurse license in Ohio, in good standing without active board restrictions
