The role is remote within Ohio, supports hospice quality processes, and requires independent prioritization, strong communication, and regulatory knowledge.

Provides hospice and end-of-life nursing care using the nursing process, including assessment, planning, intervention, implementation, and evaluation. Collaborates with patients, families, and interdisciplinary teams while documenting care and maintaining professional nursing standards. This full-time extended-hours role involves three 12-hour overnight shifts weekly, every other weekend, holiday coverage, and driving throughout Columbus.

Supports quality, education, competency, and continuous performance improvement for Care Coordination Center operations. Reviews calls and documentation, analyzes Genesys and patient satisfaction data, identifies trends, develops training and onboarding, supports coaching plans, standardizes workflows, audits process adoption, and partners with leadership to improve patient, family, teammate, and referral partner experiences.

Provide safe, non-emergency medical transportation for hospice patients: operate and maintain transport vehicle, document trips and reports, assist other departments, participate in quality improvement, and train/precept new staff. Ensure compliance with laws, maintain required training and screenings, and support patient care needs.

Designs, implements, and manages compensation programs, maintains salary grades and HRIS job data, performs market benchmarking, administers merit/incentive programs, ensures legal compliance, supports leadership with compensation recommendations, and assists HR projects and quality improvement.

Provide skilled LPN care in patients' homes under RN supervision: assessments, medication administration, documentation in EHR, patient/caregiver education, interdisciplinary collaboration, travel across service area, and participation in quality improvement and precepting.

Provide non-emergent, comfort-focused care and transportation to hospice patients. Administer first-aid within EMT scope, operate and maintain medical transport vehicle, document care, offer emotional support, participate in quality improvement, and precept new staff as needed.
