Coordinates patient referrals, obtains prior authorizations, verifies insurance eligibility, submits clinical documentation, and tracks referrals through completion. Communicates with patients, clinicians, specialists, and agencies; provides scheduling instructions, resolves referral issues, documents communications in electronic health records, and verifies that consultation and diagnostic reports are received and scanned. Requires accurate data entry, healthcare terminology knowledge, and bilingual English/Spanish communication.

Facilitates evidence-based workshops for justice-involved community members covering anger management, emotional regulation, communication, conflict resolution, and life skills. Plans and promotes educational sessions, conducts outreach, builds community partnerships, supports participants, maintains relationships, documents activities, and collaborates with program staff. Requires bilingual Spanish-English communication, community education experience, flexible scheduling, and travel between clinic sites.

Provide direct therapeutic services, psychosocial assessments, counseling, crisis intervention, case management, treatment documentation, referrals, and client support for children, adults, couples, families, and groups. Participate in case reviews, quality assurance activities, audits, protocol reviews, and coordination with outside agencies while maintaining compliance with legal, ethical, and licensing requirements.

Supervise community health and substance use prevention programs, including work plans, budgets, staff, reporting, evaluations, audits, grant support, and community partnerships. Track allowable expenses, recommend budget revisions, facilitate meetings, coordinate with agencies and government representatives, and provide direct services when needed. The role also requires bilingual English/Spanish communication, flexible scheduling, and local travel for meetings and reporting.

Provides evidence-based lactation assessments, counseling, feeding plans, and follow-up for prenatal and postpartum patients and infants. Evaluates latch, positioning, milk transfer, and feeding concerns; coordinates breast pump orders; connects families with community resources; collaborates with multidisciplinary healthcare teams; documents care; and provides in-person and telehealth services. Participates in outreach and health education while supporting culturally responsive, patient-centered care.

Provides evidence-based lactation assessments, infant-feeding plans, breastfeeding education, counseling, and follow-up for prenatal and postpartum patients and infants. Assesses feeding concerns, coordinates breast pump orders, makes referrals, connects families with community resources, documents clinical care, and collaborates with multidisciplinary healthcare teams. Services are delivered in person and through telehealth, with participation in outreach and health education activities.

Plans, implements, and evaluates therapeutic recreational activities for PACE participants, including frail older adults and people with cognitive, developmental, or physical disabilities. Coordinates individualized care planning with interdisciplinary teams, leads dementia-specific programming, organizes outings and events, trains recreation staff, maintains records, manages supplies and budgets, and supports participants and caregivers with adaptive activities and leisure education.

Leads the PACE Quality Assurance and Performance Improvement program, ensuring compliance with CMS, state, and accreditation requirements. Oversees audits, regulatory readiness, risk assessments, incident reporting, grievances, corrective action plans, performance improvement projects, and quality reporting. Partners with medical, operations, and interdisciplinary leadership to improve care outcomes, maintain compliance, educate staff, and present quality data to executives, committees, and the Board.

Supports clinic operations by supervising staff, managing patient flow and schedules, monitoring provider productivity, maintaining quality and compliance standards, coordinating staffing and training, overseeing facility security and supplies, resolving patient concerns, and completing operational reports. Requires collaboration across front and back office teams, bilingual English/Spanish communication, and knowledge of community health center programs and regulations.

Coordinates and supervises clinic programs, overseeing work plans, budgets, staff, reporting, program objectives, evaluations, meetings, and grant-related development. Tracks fiscal compliance, recommends budget revisions, facilitates internal and community meetings, and supports program implementation. Requires a related bachelor’s degree, two years of experience, English/Spanish bilingual proficiency, reliable transportation, and a California driver’s license.

Manage leave of absence, workers’ compensation, and ADA accommodation cases. Administer federal and state leave programs, determine eligibility, coordinate documentation and return-to-work processes, collaborate with payroll and third-party administrators, maintain employee privacy, and produce leave and accommodation metrics.
