Teams at Visiting Nurse Association (VNA)

Perform hospice eligibility assessments in hospitals, SNFs, and community settings; educate patients, families, and referral partners; coordinate admissions and documentation; build relationships with physicians and case managers; serve as a clinical resource and participate in on-call rotation.

Provide home-based hospice nursing care including assessments, individualized care plans, symptom management, medication management, coordination with interdisciplinary team, family education, DME arrangement, documentation, and independent caseload management across assigned counties.
