Hospice RN Case Manager – Full Time
VNA Hospice of Northwest Indiana (NWI)
Schedule: Monday–Friday, 8:00 AM–4:30 PM /On-Call: Minimum on-call rotation
Service Area: Porter, LaPorte, Jasper, Lake, Newton & Starke Counties
About the RoleVNA Hospice NWI is expanding and seeking compassionate, skilled Registered Nurses to join our interdisciplinary hospice team. As an RN Case Manager, you will coordinate and deliver high-quality hospice care to patients in their homes, ensuring comfort, dignity, and support for both patients and families.
Key Responsibilities- Provide comprehensive nursing assessments and ongoing evaluation of patient needs.
- Develop, implement, and revise individualized Plans of Care.
- Coordinate services with the interdisciplinary team (physicians, social workers, aides, chaplains, etc.).
- Educate patients and families on disease progression, symptom management, and care techniques.
- Perform physical assessments, manage medications, and ensure timely documentation.
- Arrange for necessary DME, supplies, and medications.
- Function independently while managing the demands of a dynamic caseload.
Requirements
Qualifications- Current Indiana RN license (required).
- Minimum Associate Degree in Nursing from an accredited program.
- At least one year of medical/surgical, long-term care, or home health experience preferred.
- Hospice experience strongly preferred or willingness to embrace hospice philosophy.
- Current CPR/BLS certification.
- Valid driver’s license and auto insurance.
- Sensitivity to issues of loss, grief, and end-of-life care.
