Provides direct, personalized hands-on care to residents, supports comfort and dignity, communicates needs with an interdisciplinary team, contributes to care plans, and works varied shifts including weekends, holidays, and emergency call-backs.

Provides occupational therapy services by conducting evaluations, developing patient care plans, setting short- and long-term goals, monitoring patient movement, and using proper body mechanics. Requires a current occupational therapy license and CPR/BLS certification.

Serves as the facility’s first point of contact for patients, families, and visitors. Answers and transfers calls, operates intercom and office equipment, greets and directs visitors, processes mail, performs computer work using Word, Excel, and PointClickCare, assists with administrative duties, and maintains confidentiality.

Answers telephones, directs calls, greets visitors and new patients, provides information and directions, and assists staff with administrative duties such as typing and filing.

Provides compassionate, hands-on nursing care to residents during full-time night shifts. Responsibilities include monitoring and documenting patient status and treatments, supporting residents’ physical and emotional well-being, and upholding professional and ethical nursing standards.

Provides front-desk reception for a skilled nursing and rehabilitation facility. Screens and admits visitors, answers and routes calls, orders supplies, supports resident relations, processes work orders and complaints, protects resident privacy, assists with unpaid patient billing issues and case management, and prepares admission folders. Full-time availability and working one weekend day per week are required.

Provide meal service and food preparation support, including serving meals, portion control, tray setup and delivery, dishwashing, cleaning, and assisting cooks. Maintain safety, sanitation, infection-control practices, proper food storage, waste disposal, and report hazards or incidents. Wear protective equipment when handling infectious waste.

Provides direct nursing care to residents, including medication administration, wound care, vital sign monitoring, condition assessment, care-plan support, and thorough EHR documentation. Collaborates with physicians and staff, participates in quality improvement and facility surveys, maintains a safe therapeutic environment, and may supervise personnel. Requires an active LPN license, sound judgment, strong communication, knowledge of long-term care practices, and ability to assist with resident mobility.

Provides direct restorative care to patients, supporting eating, swallowing, self-care, mobility, and independence. Assists nursing and therapy staff with treatments, transfers, exercises, equipment, dining support, documentation, temperature logs, cleaning, and equipment delivery. Requires strong communication, teamwork, emotional stability, and an active unrestricted CNA license in good standing, plus restorative or rehabilitation services experience.

Provides evening front-desk coverage at a skilled nursing facility, welcoming and assisting residents, families, visitors, and staff. Updates the daily patient census in Excel, processes credit card payments, supports departmental projects, and maintains a professional environment. Requires dependable attendance, attention to detail, customer service skills, a high school diploma, age 18 or older, and successful completion of background and drug screening.

Supervise and support nursing staff in a skilled nursing facility, manage admissions/discharges, provide direct patient care as needed, oversee medication administration and documentation, communicate with physicians and families, and develop training for nursing staff.

Partner with the Director of Nursing to lead clinical operations, mentor nursing staff, ensure regulatory compliance and quality improvement, manage staffing and performance, support survey readiness and infection prevention, collaborate with interdisciplinary teams, and assume DON responsibilities when needed.

Coordinate and develop patient care and discharge plans in a skilled nursing facility. Collaborate with physicians, nursing, families, and community agencies; ensure compliant, descriptive documentation (HIPAA); assist admissions and discharges; attend care-plan meetings to guide patient needs and transitions.

Coordinate patient admissions for a 96-bed rehabilitation facility: verify insurance, obtain pre-authorizations, collect clinical information from admitting facilities, and conduct on-site tours for prospective patients and families. Full-time day role with variable hours; background check required.

Provide direct occupational therapy services to sub-acute inpatient skilled nursing and rehabilitation patients under OT supervision, treating orthopedic, neurological, cardiopulmonary, and complex medical conditions. Follow physician referrals, comply with laws and facility standards, and collaborate with an interdisciplinary therapy team.

Prepare and serve nutritious meals, set up and clean dining areas, assist with food prep, storage, and labeling, and follow proper food handling and safety standards while providing customer service to patients and staff.

Manage patient placement referrals, conduct assessments, develop and maintain care plans, keep patient records, support audits, and perform related duties to coordinate patient care.
