Provide psychiatric rehabilitation–oriented case management for residents: build supportive relationships, help set and achieve recovery goals, coordinate care and community resources, document progress, assist with daily living skills and household upkeep, and support safe, recovery-focused residential operations.

Provide leadership and supervision for non-residential program staff; ensure safe, compliant services per ISPs; manage scheduling, site operations, budgets, activities, documentation, incident reporting, and quality assurance while supporting staff development and consumer well-being.

Coordinate staffing, hiring, onboarding, scheduling, PTO, and training for residential youth programs. Ensure proper staffing levels, support program activities, attend meetings and PQI committees, and maintain safety and facility standards.

Supervise Nurse-Family Partnership (NFP) home visitors and support staff, oversee hiring, caseload assignment, training, performance management, budgeting, community outreach, data quality and CQI efforts. Maintain program fidelity to NFP model, manage supplies and logistics, lead stakeholder meetings, and provide reflective practice supervision and field visits to support maternal/child home visitation services.

Lead and supervise Direct Support Professionals and staff, ensure services follow ISPs and regulations, manage operations, staffing, documentation, budgets, safety, and quality improvement for behavioral health programs.

Provide community-based, person-centered case management for individuals experiencing homelessness and behavioral health challenges. Connect clients to resources, develop and update recovery plans, coordinate care with clinical and community partners, complete assessments and documentation, transport clients as needed, and ensure compliance with organizational, funder, and licensing requirements.

Provide vocational assessment, job development, placement, and individualized job coaching for people with IDD. Build employer relationships, support ISP employment goals, advocate for accommodations, document outcomes, and participate in team meetings and on-call rotations.

Coordinate person-centered supports for individuals with intellectual and developmental disabilities. Conduct assessments, develop and monitor Individual Support Plans, coordinate medical and community services, ensure regulatory compliance, support staff, maintain documentation, and promote community integration and independence.

Perform functional behavioral assessments and develop individualized Behavioral Support Plans (BSP) and Restrictive Procedure Plans (RPP). Train and mentor staff and families, implement and monitor interventions, collect and analyze behavioral data, maintain documentation (SETWorks, Excel, DocuSign), participate in interdisciplinary and human rights reviews, and ensure compliance with state, federal, and HIPAA regulations.

Develop, implement, and adjust individualized Behavior Support Plans and interventions for adults with intellectual and developmental disabilities. Conduct assessments, behavior skills training, and performance feedback for caregivers. Collaborate with an interdisciplinary team and Clinical Director across community-based day programs and residential settings to reduce severe problem behaviors and increase independence.

Support childrens programming at a family shelter: supervise play, lead activities (Bright Spaces, Kidz Zone, Art Room), monitor computer lab, administer Ages & Stages screenings (ages 3-5), document services, assist parents, manage donations and events, drive company vehicle, work occasional evenings and flexible hours.

Lead admissions, intake, and initial revenue cycle functions ensuring eligibility, billing compliance, HIPAA protection, and audit readiness. Supervise intake/billing staff, train on EHR and processes, and liaise with funding/regulatory entities to maintain operational efficiency and revenue integrity.

Provide community-based support and coordinate individualized service and recovery plans. Connect participants with housing, medical, employment, and behavioral health resources, advocate for services, transport clients, maintain documentation, collaborate with clinical teams, and participate in meetings and trainings.

Manage and oversee residential homes serving adults with IDD, supervise and train staff, ensure regulatory compliance and person-centered supports, coordinate budgets, documentation, incident management, facility and vehicle maintenance, and participate in care planning and community activities.

Provide assessments, individualized treatment plans, therapeutic interventions, crisis intervention, case management, and community-based services for adults with severe mental illness on an ACT team; coordinate resources, ensure documentation and regulatory compliance, support quality improvement, and provide clinical supervision and training.
