Supervise and coach case management staff, provide direct case management as needed, ensure compliance with program policies and funding regulations, manage client documentation and reporting (OHS/HUD), coordinate challenging cases and staff training, and perform administrative duties including data entry and resident coordination.

Provide community-based case management: assess needs, develop and implement individualized recovery/service plans, connect participants to housing, medical, employment and behavioral health resources, transport clients, document services, advocate for participants, and collaborate with clinical teams to improve outcomes.

Support participants in the Urban Youth Collaborative program by facilitating art, multimedia, and community activities aligned with individual ISP goals; assist with day trips, ADLs, classes, and engage residents at studio and home; attend trainings and follow policies.
