Provide community integration supports for adults with developmental disabilities by planning and facilitating daily outings, supervising up to three participants, implementing ISPs and behavior plans, assisting with ADLs and medication as needed, managing outing budgets and receipts, completing progress notes and paperwork, using personal/program vehicles, and completing required training.

Provide person-centered case management by assessing participant needs, developing and monitoring individualized service plans, coordinating care with providers and community resources, advocating for access to services, documenting progress and compliance, and supporting recovery and independence through community-based engagement.

Provide person-centered nursing care including assessments, medication administration (including IM injections), care coordination, health education, crisis intervention, documentation, and collaboration with multidisciplinary teams to support recovery, wellness, and access to integrated physical and behavioral health services.
