Coordinate and supervise care coordinators and resource coordinators to organize patient care, develop and monitor treatment plans, ensure regulatory compliance, track outcomes and reporting, facilitate team meetings and off-site provider coordination, provide weekly individual support and group facilitation, and assist participants with medical/psychiatric appointments.

Coordinate and oversee residential and community services for individuals with developmental disabilities. Manage a caseload (up to 30), perform monthly home visits, complete assessments and licensing paperwork, attend ISP and waiver meetings, coordinate evaluations and medical appointments, draft/update behavior plans, train and supervise assistant coordinators and staff, and act as liaison with supports coordinators and agencies.

Provide overnight, direct support to residents with mental health or IDD diagnoses, facilitating individual and group counseling, independent-living skill building, service planning, medication management, documentation, and community integration while managing a small caseload and ensuring program compliance.
