Provides direct and indirect nursing care to PACE participants through assessments, medication administration, treatments, triage, health education, care transitions, telehealth, home health, and emergency response. Collaborates with physicians and interdisciplinary team members on care planning, complex clinical situations, end-of-life support, and quality improvement. Coordinates LVN and MA activities, documents care in Epic, supports enrollment, and may provide after-hours on-call coverage.

The PACE Enrollment Specialist educates potential participants, caregivers, and referral sources about PACE services; conducts screenings and phone intakes; gathers medical, social, demographic, and eligibility information; schedules assessments; manages level-of-care enrollment processes; presents cases to the interdisciplinary team; provides center tours and community presentations; develops referral relationships; supports outreach campaigns; and documents enrollment activities in Epic.

Lead and execute integrated, multi-channel marketing strategies to drive enrollment, engagement, revenue, and brand visibility. Manage marketing staff and external partners, oversee content, CRM and analytics, allocate budget and resources, report performance to leadership, and ensure cross-departmental alignment and brand governance.

Builds community partnerships, expands patient resources, coordinates the volunteer program, leads outreach and focus groups, represents NEMS in coalitions, educates stakeholders, and supports policy, communications, press conferences, and media relations across the San Francisco Bay Area.

Supports implementation, configuration, maintenance, troubleshooting, testing, and upgrades of Epic healthcare applications. Provides first-level end-user support, collaborates with clinicians, administrators, IT teams, vendors, and project stakeholders, translates business requirements into system configurations, develops training materials and documentation, and communicates project updates. Rotational after-hours and Saturday support may be required.

Lead and supervise multidisciplinary rehabilitation and recreational therapy services across PACE sites. Responsibilities include staffing, training, performance management, regulatory compliance, QI initiatives, vendor management, emergency preparedness, data monitoring, coordination with clinical teams and external providers, Epic workflow support, and ensuring participant safety and satisfaction.

Performs phlebotomy, patient registration, reception, billing, payment collection, and accurate collection instruction for urine, sputum, feces, and semen specimens. Reviews and releases laboratory orders, posts charges, and provides patient information. Requires a California CPT-1 certificate, Basic Life Support documentation, high school diploma or GED, computer proficiency, and Cantonese or Mandarin ability. This is a part-time, non-exempt Saturday position.

Supervise back-end revenue cycle staff to ensure accurate, timely payment posting, denial follow-up, self-pay management, AR aging resolution, and month-end closing. Coordinate with front-end billing, use Epic PB for workflows and reports, handle patient and payer inquiries, train staff, and support revenue cycle management and reporting.

Senior supervisor for front-end revenue cycle overseeing coding, provider enrollment/credentialing, charge review, and claims scrubbing. Ensures accurate coding, clean-claim submission, denial prevention, staff training, performance reporting, Epic system coordination, and process improvements to accelerate cash flow and reduce denials.

Review, audit, investigate and adjudicate professional medical claims to established guidelines (Medi-Cal/Medicare), meet production targets, correct processing errors, respond to provider/member inquiries, support delegation oversight audits and report issues to supervisors and system teams.

Greet and check in patients, schedule appointments, verify insurance, collect co-payments, manage patient communications (phone and portal), assist with MyChart and outreach, update registration and demographic data, comply with HIPAA, and rotate coverage across clinic sites.

Designs and governs enterprise Zero-Trust security architecture across on-premises, clinic, and cloud environments. Leads IAM, endpoint protection, EDR, patching, network segmentation, security policy development, risk and vulnerability assessments, incident response, SOC coordination, and compliance with NIST and HIPAA. Mentors team members and liaises with external vendors and auditors.

Field-based liaison promoting and educating community providers about the MSO network and managed care services. Conducts in-person outreach, orientations, trainings, negotiates contracts, responds to complex provider inquiries (claims, authorizations, appeals), communicates DHCS/CMS policy changes, maintains provider relationships, documents outreach, and collaborates internally to resolve issues and implement audits.

Support execution and improvement of the PACE Compliance Oversight Program by performing internal audits, monitoring vendor and regulatory compliance, managing corrective action plans, assisting with HPMS submissions and responses, conducting regulatory research, updating policies/procedures, and coordinating with stakeholders to ensure timely regulatory compliance.

Manage moderate-to-high complexity IT projects across infrastructure, cloud, network, and systems. Own project artifacts, vendor coordination (Rackspace, Palo Alto, Cisco, Azure), stakeholder engagement, BC/DR workstreams, clinic technology rollouts, budget tracking, and PMO standards. Support program leads and mentor coordinators.

Lead and operate the enterprise IT PMO, govern a complex portfolio (infrastructure, EHR/Epic, AI), establish governance and reporting, supervise PMO staff, manage vendor relationships, and ensure portfolio delivery on scope, schedule, and budget while aligning to organizational goals.

Process and track PACE medical referrals end-to-end: verify eligibility/coverage, obtain prior authorizations, communicate with providers/hospitals, collect consultation notes, document activities in Epic EHR, and ensure referrals meet turnaround times while coordinating with care teams.

Manage and supervise social work, homecare, and community nursing across PACE sites, ensuring staffing, contractor oversight, discharge coordination, LOC submissions, regulatory compliance, quality improvement, participant satisfaction, and Epic workflow expertise. Drive retention, RCFE admissions, grievance resolution, audits, and cross-disciplinary coordination with providers and families.

Provides administrative support to the MSO Value Based Care team: triages phone/mailbox inquiries from providers, vendors and clients on claims, authorizations, and benefits; coordinates grievance investigations and follow-up with health plans; assists outreach and patient care improvement activities; maintains timely email/voicemail responses and performs other clerical tasks as assigned.

Lead full-stack development of scalable EHR systems using TypeScript, React, and NextJS. Architect APIs and secure identity solutions (OAuth/SAML/OpenID), optimize PostgreSQL and MS SQL Server schemas and queries, enforce containerized deployments (Docker) and CI/CD, mentor engineers, integrate FHIR/HL7 interoperability, collaborate with clinical and operations teams, and ensure HIPAA-compliant, production-grade delivery.
