Teams at North East Medical Services (NEMS)

Reviews utilization management cases, treatment authorization requests, inpatient stays, and discharge plans to ensure medically necessary, timely, and appropriate care. Coordinates transitions, follow-up services, community resources, and post-discharge support with providers and interdisciplinary teams. Documents case activity, supports audits and reporting, guides UM staff, applies clinical judgment, and collaborates with medical directors, case managers, patients, and families to improve outcomes and resource utilization.

Provides direct and indirect nursing care to PACE participants in clinic, home, telehealth, and telephone settings. Conducts assessments, administers medications and treatments, performs triage, coordinates care transitions, supports interdisciplinary care planning, educates participants and caregivers, and responds to emergencies. The RN also coordinates LVN and MA activities, documents care in Epic, supports quality improvement, enrollment, end-of-life care, and may provide after-hours on-call coverage.

Provides reception and administrative support for a PACE healthcare center. Greets participants and visitors, answers calls, manages messages, schedules and tracks appointments, processes mail, coordinates resources and meetings, maintains supplies, runs operational reports, and supports communication between field staff and the center. Uses Epic as a workflow expert and assists with issue resolution. Requires bilingual English and Cantonese fluency, computer proficiency, BLS and First Aid certification, and strong organizational and customer service skills.
