Assist in planning and running group and individual activities for senior residents, provide companionship and transportation support, help with nursing-area tasks, maintain records for Adult Day Services, assist with meals and daily living, and complete required trainings and certifications.

Provide individualized disaster case management to survivors: intake, needs assessments, recovery planning, resource coordination, documentation in SharePoint, and collaboration with Long Term Recovery Groups and partner agencies.

Supervise and support direct service staff, recruit/hire/train/evaluate personnel, ensure program compliance with agency/state/federal standards, coordinate services and referrals, and provide case management as needed.

Provide trauma-informed individual and group counseling, intake assessments, crisis de-escalation, referrals, and case collaboration for unaccompanied children; maintain timely documentation and adhere to licensing and agency policies.

Provide individual and family therapy using evidence-based interventions and the Collaborative Problem Solving model; participate in IAFT and clinical team meetings; complete assessments and maintain progress/therapy notes in Kaleidacare; ensure confidentiality, comply with licensing and agency policies, complete continuing education, and perform occasional in-person visits in the Winston-Salem area.

Prepare and produce patient meals following food safety regulations and dietary needs, maintain kitchen cleanliness, participate in training, collaborate with healthcare staff, and provide general support within the dietary department.

Supervise disaster case management staff, provide training and technical support, manage client intake and complex cases, maintain case records and databases (CAN/EMR), conduct file reviews, coordinate community resources, report on program metrics, and support outreach and recovery planning.

Assist with meal preparation and service for residents, maintain dietary area cleanliness and food safety, follow recipes and instructions, set up dining service, interact compassionately with residents, and attend trainings.

Process and adjudicate provider claims (paper and electronic) per policy; verify Medicare and contract rates; research and resolve discrepancies and system edits; communicate with providers and internal teams to correct authorizations; pay adjudicated claims, maintain claims files, and record annual 1099s.

Provide direct resident care in a long-term care setting: administer and document medications (oral, IM, SC, G-tube, IV), follow individualized care plans and LSC procedures, perform wound/ostomy/catheter care, respiratory treatments, assist with admissions/discharges, report condition changes/incidents, supervise CNAs/medication aides, participate in education and infection control, and maintain accurate electronic medical records.

Manage day-to-day operations of a housing-focused program, conduct resident check-ins and inspections, provide life-skills coaching and social support, document incidents and quality assurance, coordinate community resources and meetings, offer on-call emergency support, and represent the agency to partners.

Provide comprehensive clinical services including intake assessments, individual and group counseling, screenings, referrals, crisis de-escalation, documentation in Extended Reach, and participation in multidisciplinary team meetings while maintaining confidentiality and adhering to licensing and agency policies.

Prepare and produce nutritious meals following food safety regulations and patient dietary needs. Participate in training, maintain kitchen cleanliness, collaborate with healthcare staff, and provide general support within the dietary department to meet patient nutrition goals.

Manage daily operations of a Raynor Street group home, ensuring licensing, COA standards, budgets, documentation quality, staff supervision, vehicle/facility maintenance, incident processing, EMR note review, community representation, and occasional direct care or driving as needed.

Provide nursing care to senior residents under physician and DON supervision: administer and document medications/treatments, maintain drug security, monitor and report condition changes, perform catheter care, dressings, respiratory treatments, and supervise nursing staff.

Provide trauma-informed individual and group counseling to youth, perform assessments and crisis de-escalation, document progress in Kaleidacare, participate in child/family and Day Services team meetings, make referrals, maintain confidentiality, comply with licensing and continuing education requirements, and collaborate with DSS as needed.

Provide community-based, trauma-informed support to veterans—assisting with activities of daily living, promoting independence, coordinating care and transportation, developing individualized service plans, documenting progress, engaging veterans in community activities, and supporting crisis response and safety planning.

Provide direct resident care in a senior living community, including safe medication and treatment administration, documentation in the electronic medical record, reporting changes or incidents, participating in care plans, and following facility policies and state requirements under RN/physician supervision.
