Leads compliance, quality improvement, education, data analysis, medical records oversight, and survey readiness for home health and hospice programs. The role audits documentation, develops performance dashboards, coordinates regulatory reviews, trains staff, and partners with clinical and operational leaders to improve compliance, workflows, accountability, and patient-care outcomes.

Provides grief counseling and bereavement services to hospice patients’ families, caregivers, and staff. Assesses emotional risk, develops care plans, coordinates referrals, supports program development, participates in interdisciplinary hospice activities, documents services, and ensures Medicare compliance. The role also provides community grief and loss services and requires independent work, strong communication, counseling expertise, and collaboration with hospice teams.

Coordinates and leads life enrichment activities primarily for memory support residents, including exercise, music, arts, cognitive games, cooking, and outings. Develops schedules, manages sign-ups, prepares event documentation, supports resident safety, collaborates with staff and families, and completes administrative tasks. Provides one-on-one programming, department coverage, and feedback on resident participation while maintaining a positive, supportive environment.

Lead and manage social services for a long-term care facility, overseeing resident advocacy, discharge planning, psychosocial assessments, individualized service plans, care coordination, compliance, documentation, and interdisciplinary collaboration to enhance residents' quality of life.
