We are seeking an experienced and compassionate Director of Social Services to join our leadership team. This full-time leadership position plays a vital role in enhancing the quality of life for residents by overseeing social services, resident advocacy, care coordination, and discharge planning within our long-term care community.
The ideal candidate is an experienced long-term care professional who is passionate about resident-centered care, skilled in navigating complex healthcare needs, and committed to supporting residents and families throughout their journey.
What You'll Do
As Director of Social Services, you will:
- Lead and oversee all social services operations within the facility.
- Champion resident rights, dignity, independence, and quality of life.
- Coordinate discharge planning and care transitions to promote positive outcomes.
- Partner with interdisciplinary care teams to ensure comprehensive, person-centered care.
- Conduct psychosocial assessments and develop individualized service plans.
- Provide support, counseling, education, and guidance to residents and family members.
- Connect residents and families with community resources and support services.
- Facilitate care plan meetings, family conferences, and resident advocacy initiatives.
- Ensure compliance with federal, state, and professional standards.
- Maintain accurate and timely documentation and records.
- Contribute to quality improvement efforts and organizational excellence.
What You'll Bring
Required Qualifications
- Previous experience serving as a Director of Social Services in a Long-Term Care setting
- Education: Master's-level social work preparation (MSW-prepared)
- In-depth knowledge of long-term care regulations, resident rights, care planning, and discharge processes.
- Strong leadership, communication, and relationship-building skills.
- Ability to work effectively with residents, families, physicians, and interdisciplinary team members.
- A commitment to compassionate, resident-focused care and advocacy.
