Obtains and monitors insurance authorizations for physician-ordered pediatric home health services. Maintains authorization workflows, contacts payers, processes original and renewal authorizations, submits required documentation, schedules evaluations, processes payer changes and discharges, and assists with referrals. The role requires bilingual English/Spanish communication, accurate database and Microsoft Office use, multitasking, and timely follow-up with families, clinicians, physicians, and insurance payers.

Processes pediatric therapy referrals, verifies eligibility and documentation, obtains Medicaid authorizations, creates client records, coordinates therapist staffing, and maintains communication with families, therapists, and physicians. Manages client holds, discharges, certifications, referral workflows, and processing reports while supporting timely initiation of home health services.

Provides pediatric home-health case management, including patient assessments, care planning, coordination, education, supervision of field nurses, and evaluation of skilled nursing services. The role conducts start-of-care and recertification visits, collaborates with interdisciplinary teams, participates in on-call rotations, and ensures compliance with physician orders and applicable regulations. Regular driving, patient transfers, equipment handling, and physical assistance are required.

Provides pediatric home health case management, including patient assessments, care planning, supervision of field nurses, care coordination, patient and family education, recertification, and compliance with physician orders and regulations. Participates in interdisciplinary meetings, staff training, scheduling, delegation, and on-call rotation. Requires regular driving, physical patient assistance, and collaboration with healthcare professionals to support quality outcomes.
