Responsibilities

The Nursing Case Manager is responsible for the delivery and supervision of patient care through the process of assessment, diagnosis, planning, implementation, evaluation and coordination of care. 


Essential Duties and Responsibilities 


• Provide case management and coordination of skilled nursing care to patients requiring intermittent professional nursing services.• Perform Start of Care and Recertification for patients.• Perform hospital assessment of new patients• Establish a plan of care and communicate to direct patient care staff.• Monitor and supervise the nursing care of patients within his or her territory.• Attends all IDT and mandatory meetings, as well as staff meetings, team conferences, and educational in-services.• Educate patients and their families on specific details of diagnosis, treatment and prevention.• Participate in an on-call rotation as a member of the case management team.• Collaborate with other professional disciplines to ensure that patients are receiving quality care, necessary to achieve and exceed individual goals.• Provide these services in accordance with physician orders in adherence to state, federal, and national accreditation commission regulations. Under the direction and supervision of clinical supervision staff and agency administrator.• Participate in all aspects of supervision of field staff including but not limited to: training, delegation, scheduling, staff assignment, and discipline.• Adhere to productivity guidelines listed on "SOP - Productivity".• Perform other duties as assigned.

Qualifications

Education and Work Experience Requirements

  • Current State Registered Nursing License or compact license where applicable.
  • Home Health Case Management Experience preferred.
  • Two (2) Years experience as RN in clinical care setting preferred.
  • KanTime EMR Experience preferred.
  • Current CPR Certification.
  • Valid state driver license.

Other Qualifications and Skills

  • Ability to read and write in simple correspondence with effective public speaking in small groups.
  • Excellent critical thinking and critical thinking skills.
  • Detail-oriented with strong organizational skills.
  • Ability to interpret instruction with the ability to solve practical problems with a variety of concrete variables.
  • Proficient computer skills to include Microsoft Office Suite, record keeping and routine database activity.
  • Manage multiple tasks simultaneously.

Driving Responsibilities: This position requires driving on behalf of the company. Qualified applicants and team members driving personal vehicles while performing job duties for KidsCare will: 

  • Be at least 18 years of age.
  • Possess and maintains a valid state driver’s license. 
  • Have at least one (1) year of US driving experience.
  • Not have engaged in (or have been cited for) any of the following or similar behaviors within the last five (5) years:
    • Driving while impaired or driving while in possession of a controlled substance for which the driver does not have a current valid license or prescription.
    • Failure to comply with implied consent law (refusing a test for impairment),
    • Committing a Misdemeanor or Felony involving a motor vehicle. KidsCare Home Health adheres to all federal and state mandates regarding the review of MVR checks and has an established policy for reviewing and adjudicating any convictions or charges found. As a result, KidsCare reserves the right to suspend or separate employment strictly based upon the information provided and its potential impact upon organizational liability related to performing specific job required responsibilities. 
    • Showing a pattern of dangerous driving habits in MVR Records  
    • Failing to follow law enforcement instruction
  • Not having more than three (3) minor moving violations. Examples include speeding, failure to yield the right of way, unsafe lane changes, child safety seat violations, etc.
  • Agree to regular driving history searches as requested by the company (refusal to submit to driving history searches may be grounds for separation). (NOTE: Individuals receiving company owned (fleet vehicles) may have additional requirements. MVRs will be reviewed during the fleet process.
  • In addition to the above, the company may, in its sole discretion, determine other behaviors as “at-risk” which may lead to revocation or restriction of company driving privileges. 

Physical Demands/Working Environment

  • Position regularly requires bending, reaching, standing, stooping, sitting, twisting, talking, and hearing.
  • Push and pull 50 pounds (position patient, move equipment, etc.).
  • Support 50 pounds of weight (ambulate patient).
  • Lift 50 pounds (pick up a child, transfer a patient, etc.) as well as assist with weights of more than 100 pounds.
  • Carry equipment and supplies.
  • See information up to 24 inches away (monitors, computer screens, etc.).

Skills Required

  • Current state Registered Nursing license or compact license where applicable
  • Current CPR certification
  • Valid state driver's license
  • At least one year of United States driving experience
  • Home health case management experience
  • Two years of RN experience in a clinical care setting
  • KanTime EMR experience
  • Proficiency with Microsoft Office Suite, record keeping, and routine database activity
  • Strong critical thinking, organizational, communication, and multitasking skills
  • Ability to meet physical requirements, including lifting and supporting up to 50 pounds and assisting with weights over 100 pounds
