Manages a caseload of justice-involved adults with substance use and behavioral health conditions during reentry from incarceration. Conducts assessments, develops individualized service plans, coordinates treatment and recovery supports, facilitates medication-assisted treatment continuity, and connects clients with housing, employment, transportation, benefits, and healthcare. Collaborates with correctional, clinical, community, and probation partners; monitors progress; maintains documentation; transports clients; and provides services across worksites.

Provides clinical leadership and operational oversight for a recovery and reentry program serving justice-involved individuals with substance use and co-occurring behavioral health conditions. Supervises clinical staff, peer recovery specialists, and case managers; ensures evidence-based care, quality assurance, clinical fidelity, interdisciplinary coordination, and continuity from incarceration through community reintegration. Collaborates with correctional staff, courts, probation and parole, healthcare providers, and community partners. Provides clinical supervision, responds to emergencies, supports program implementation, and travels among worksites and counseling locations.

Provides behavioral health assessment, diagnosis, treatment planning, counseling, crisis intervention, and care coordination for justice-involved adults with substance use and co-occurring mental health conditions. Delivers trauma-informed services in correctional and community settings, supports reentry and recovery, coordinates with interdisciplinary providers, completes clinical documentation, and may transport clients. Requires flexibility for modified schedules, behavioral intervention training, and travel among worksite locations.

Conducts financial interviews to determine accurate billing based on available resources. Verifies Medicaid, Medicare, commercial insurance, and other coverage; assists customers with billing questions; records accurate data and documentation; follows social services and insurance regulations; and provides administrative support across Horizon worksites as needed.

Provides school-based mental health services, including client assessments, treatment planning, counseling, community-resource education, advocacy, and coordination of support services. Requires a qualifying 60-hour master’s degree in counseling or a related discipline and licensure eligibility or residency. No prior experience is required. The clinician must obtain clinical supervision, maintain a valid driver’s license, accommodate modified schedules, and travel between Horizon worksites as needed.

Develop and implement recruitment strategies for Horizon Behavioral Health. Source, interview, assess, and secure qualified candidates while partnering with hiring managers and HR leadership. Build talent pipelines, analyze recruitment metrics, generate reports, support workforce development, attend career fairs, and provide recruitment support across Horizon locations. The role requires strong communication, presentation, interviewing, organizational, and social-media recruiting skills, with occasional travel to work sites and meetings.

Provides hands-on desktop and computer support by receiving, prioritizing, documenting, diagnosing, and resolving end-user help requests. Maintains proper computer operations, supports Windows and Microsoft Office applications, delivers customer service, and assists users across organizational sites. The role may provide administrative support, attend trainings and meetings, work modified schedules, and travel between Horizon locations as operational needs require.

Provide assessments, develop and implement individualized treatment plans, deliver individual/group/family counseling, conduct risk and clinical interviews, educate on community resources, and participate in trainings. May transport clients and travel between Horizon worksites; must complete behavioral intervention and de-escalation training.

Provide entry-level case management for individuals and families: assess needs, coordinate referrals and services, document in EMR, perform follow-up, make home visits, transport clients, and participate in required trainings under supervision.

Provide one-on-one peer mentoring, lead peer support groups, conduct outreach, assist clients accessing community resources, transport clients as needed, and complete required training/supervision to pursue CPRS certification within one year.

Oversee and supervise case management teams across multiple VA sites, manage team leaders and direct-line case managers, ensure clinical program quality, implement treatment and assessment standards, and coordinate community stakeholders and outcomes measurement.

Lead a multidisciplinary FEP team delivering assessment, treatment, rehabilitation, and supervision. Manage daily clinical operations, budgeting, documentation review, licensed clinical supervision, training compliance, travel between sites, and behavioral intervention duties. Grant-funded role.

Provide school-based mental health services: assess referred students, develop and implement treatment plans, conduct individual/group/family therapy, perform risk assessments, coordinate community resources, document care, and pursue required clinical residency/supervision.

Provide in-home and community-based Multi-Systemic Therapy (MST) to youth ages 11–17, perform assessments and diagnoses, coordinate care, deliver crisis intervention and safety planning, participate in on-call rotation, and work with the MST team under clinical supervision.

Supervise a multidisciplinary mobile crisis team of ~10 clinicians responding to community mental health crises. Support Clinical Director in developing mobile response programming, provide clinical oversight, staff supervision, performance management, risk assessment, and crisis decision-making. Manage budgets, coordinate trainings, travel between sites, transport clients as needed, and ensure staff complete required behavioral intervention training.

Provide assessments, develop and implement individual treatment plans, deliver individual/group/family counseling, perform risk assessments, educate clients on community resources, and participate in clinical supervision and behavioral intervention training. Travel between sites and occasional nonstandard schedules required.

Provide outpatient clinical assessments, develop and implement individualized treatment plans, deliver individual/group/family counseling, perform risk and substance abuse assessments, educate on community resources, and participate in trainings and supervision/residency requirements.

Provide mobile outreach, home visits, crisis intervention, short-term therapy, and supportive counseling as part of an ACT multidisciplinary team for rural clients with serious mental illness. Perform psychiatric assessments, participate in behavioral intervention training, travel to sites, and apply for required clinical residency/supervision within 30 days of hire. Grant-funded through April 2029.

Provide community mental health services including mobile outreach, home visits, crisis intervention, short-term focused therapy, supportive counseling, and psychiatric assessments. Complete documentation, participate in trainings, travel between sites, and apply for required clinical supervision/residency. Position is grant-funded and may end with grant expiration.

Manage and supervise the Developmental Disabilities Case Management team across multiple counties; oversee team leaders and case managers, ensure clinical supervision, program administration, outcome measurement, and community collaboration; maintain licensing, training, and operational compliance.
