Teams at Horizon Behavioral Health

Manages a caseload of justice-involved adults with substance use and behavioral health conditions during reentry from incarceration. Conducts assessments, develops individualized service plans, coordinates treatment and recovery supports, facilitates medication-assisted treatment continuity, and connects clients with housing, employment, transportation, benefits, and healthcare. Collaborates with correctional, clinical, community, and probation partners; monitors progress; maintains documentation; transports clients; and provides services across worksites.

Provides clinical leadership and operational oversight for a recovery and reentry program serving justice-involved individuals with substance use and co-occurring behavioral health conditions. Supervises clinical staff, peer recovery specialists, and case managers; ensures evidence-based care, quality assurance, clinical fidelity, interdisciplinary coordination, and continuity from incarceration through community reintegration. Collaborates with correctional staff, courts, probation and parole, healthcare providers, and community partners. Provides clinical supervision, responds to emergencies, supports program implementation, and travels among worksites and counseling locations.

Provides behavioral health assessment, diagnosis, treatment planning, counseling, crisis intervention, and care coordination for justice-involved adults with substance use and co-occurring mental health conditions. Delivers trauma-informed services in correctional and community settings, supports reentry and recovery, coordinates with interdisciplinary providers, completes clinical documentation, and may transport clients. Requires flexibility for modified schedules, behavioral intervention training, and travel among worksite locations.
