Serve as the primary liaison between hospital discharge teams, JV hospice/home health agencies, patients, and families to coordinate timely transitions to home health or hospice. Evaluate referrals, educate staff and families, obtain consents, document in EMR, facilitate admissions and handoffs, support GIP hospice program expansion, track referral outcomes, and participate in value-based care coordination initiatives.

Designs, facilitates, and manages end-to-end leadership development programs. Coaches leaders, runs cohorts and events (virtual/in-person), creates practical learning content, tracks outcomes, and partners with stakeholders to improve leader capability, engagement, and trust.

Lead enterprise operational transformation by designing workflow requirements, guiding vendor selection and configuration, establishing configuration standards, defining performance measures, integrating vendor data, piloting and scaling technology-enabled operating models, and partnering with deployment leadership to ensure consistent, scalable execution across a national home-health platform. Travel ~35%.

Provide hands-on home health nursing including medication management, wound care, and health monitoring. Use AI-supported tools to streamline visit documentation. Travel locally across assigned territory, work full-time 40 hours/week with one weekend rotation, and focus on patient interaction and quality of care.
