Serve as the primary liaison between hospital discharge teams, JV hospice/home health agencies, patients, and families to coordinate timely transitions to home health or hospice. Evaluate referrals, educate staff and families, obtain consents, document in EMR, facilitate admissions and handoffs, support GIP hospice program expansion, track referral outcomes, and participate in value-based care coordination initiatives.

Perform HR-supported, student-driven strategic projects across Talent Acquisition, Colleague Relations, Compensation, Benefits, Learning & Development, and HRIS. Assist with departmental materials, collaborate with HR leaders, communicate findings to directors, manage multiple tasks, document program initiatives, provide outstanding customer service, and complete other assigned duties while receiving mentorship.

Serve as the hospital-based RN liaison to coordinate and facilitate home health and hospice admissions and transitions. Assess referrals, educate hospital staff and families, document in EMR, coordinate discharge planning, arrange services and consents, and promote Value-Based care initiatives while building relationships with post-acute providers and internal teams.
