Provide benefits support to adults with severe mental illness: assist with applications and renewals for Medicaid, Social Security, APTD, WIC; set up NH Easy accounts; gather documentation; coordinate with DHHS, SSA and treatment teams; document services; provide follow-along support and participate in consultations and community meetings.

Provide high-level administrative and project support to the Senior VP/COO and senior leadership, manage calendars, coordinate meetings and travel, take meeting minutes, handle confidential information, assist program directors, lead special projects, and support operations initiatives across the agency.

Provide community-based case management for children and families including assessments, care planning, benefits coordination, resource linkage, crisis intervention, documentation, family training, and collaboration with treatment teams to support recovery and age-appropriate functioning.

Provide peer-based crisis support and counseling using lived experience on a mobile crisis team. Link clients to community resources, follow trauma-informed care and SAMHSA competencies, document in EHR, collaborate with clinical team, and perform follow-up after crises.

Provide outpatient clinical services to children and families including diagnostic assessment, treatment planning, individual/family/group therapy, crisis intervention, documentation, eligibility assessments for state-funded services, collaboration with treatment teams and external providers, and meeting billable productivity expectations. Work primarily in community/home settings with some telehealth and office-based work.

Provide community-based assessment, care planning, benefit coordination, counseling, crisis intervention, and family training for children and adolescents. Monitor services, document in EHR, collaborate with treatment team and external providers, and meet productivity and billable service expectations.
