Provide benefits support to adults with severe mental illness: assist with applications and renewals for Medicaid, Social Security, APTD, WIC; set up NH Easy accounts; gather documentation; coordinate with DHHS, SSA and treatment teams; document services; provide follow-along support and participate in consultations and community meetings.

Provide community-based case management for children and families including assessments, care planning, benefits coordination, resource linkage, crisis intervention, documentation, family training, and collaboration with treatment teams to support recovery and age-appropriate functioning.

Provide outpatient clinical services to children and families including diagnostic assessment, treatment planning, individual/family/group therapy, crisis intervention, documentation, eligibility assessments for state-funded services, collaboration with treatment teams and external providers, and meeting billable productivity expectations. Work primarily in community/home settings with some telehealth and office-based work.
