Plans and executes complex healthcare projects from initiation through closure. Responsibilities include defining objectives, gathering requirements, managing resources and budgets, coordinating stakeholders, analyzing performance, tracking risks and issues, maintaining documentation, and ensuring deliverables meet quality standards. The role also develops project dashboards, milestone reports, ROI analyses, and success metrics while supporting project management methodologies and operational process improvements.

Leads strategic planning, operational initiatives, and project execution across physician practices, agilon health, payers, and enterprise teams. Improves care quality, efficiency, growth, patient experience, and organizational effectiveness through process improvement, technology adoption, metric tracking, ROI analysis, and innovative care models. Establishes performance objectives, monitors results, drives change, and supports integration across healthcare partners.

Provide case management for high-risk patients across inpatient, ambulatory, and post-acute settings. Perform assessments, develop and implement Plans of Care and self-management plans, coordinate with PCPs and interdisciplinary teams, facilitate referrals and community resource use, perform home visits as needed, document interventions and outcomes, and ensure regulatory and quality standard compliance (HEDIS, NCQA, URAC).
