Position: Referral Coordinator

Wage: $19.00 to $22.00 per hour (Compensation is determined based on relevant experience, skills, and qualifications, with most new hires starting between the minimum and midpoint of the posted wage range)

Hours: Part-Time, Up to 25 hours per week

Location: YMCA Association Office, Tacoma, WA

 

As a Referral Coordinator, you’ll play an important role on our Care Coordination team, connecting individuals to opportunities that support better health and well-being. You’ll reach out by phone and email to help referred individuals navigate enrollment, answer questions, and connect with resources such as chronic disease programs, resource navigation services, YMCA membership, facility access, and more. You’ll also maintain accurate records, support program tracking and reporting, and provide administrative support for our Health Initiative services. Above all, this role is focused on creating a positive, supportive experience for every participant.

 

Qualifications:

  • One more year of education and/or work experience in health promotion, public health, community outreach, or a similar field.
  • Care Management skills and/or Motivational Interviewing skills are preferred.
  • Working knowledge of preventive health programs and philosophy
  • Intermediate to advanced computer skills and experience with Microsoft Office.
  • Ability to work independently and with a team.
  • Comfortable interacting with individuals’ health-related information and communicating with health care professionals.
  • Positive attitude and previous experience with diverse populations.
  • Must be able to pass Background and Reference checks (in accordance with the WA State Fair Chance Act).
  • Committed to valuing and promoting diversity and contributing to an inclusive working and learning environment.
  • Able and willing to complete online Child Abuse Prevention training on first day, and other online and in-person training as required.

 

Key Responsibilities: 

  • Maintain electronic database of health information and enter and sends/receives data through referral management system following all privacy and security procedures and protocols.
  • Conduct care coordination conversations with referred individuals and other prospective community members utilizing motivational interviewing and active listening skills.
  • Support connecting individuals to various YMCA services including but not limited to evidence-based chronic disease programs, resource navigation services, center programming, membership as well as resources outside of YMCA when applicable.
  • Ensure the program/enrollment targets are met and consistent with program standards.
  • Assists with marketing and retention activities to create robust classes with high enrollment.
  • Ensures accurate intake forms, data collection forms and appropriate program documentation are completed.
  • Supports timely and accurate data collection and reporting for Health Initiative Department.
  • Follows all privacy and security policies and protocols.
  • Maintain open communication and work proactively to build strong relationships with all staff and members.
  • Other duties as assigned.
