We are seeking a PRN LPN to provide nursing support to clients within our agency. This role is critical to meeting current care plan requirements, skilled nursing in a home seeting. The Licensed practical nurse performs assessments and simple medical nursing procedures in client's homes, under the guidance of the RN Supervisor, in accordance with physician's orders.

Hours: self-scheduled during the week. Monday through Friday.

Responsibilities and Duties Include:
- Medication set-ups and reviews
- Skin assessments and general health evaluations
- Basic nail care
- Client education and documentation
- Enable patients to stay independent while they remain in their own homes.
- Perform routine supervisory visits
- Follow up for unskilled services
- Maintain a high degree of confidentiality at all times due to access to sensitive information.
- Maintain regular, predictable, consistent attendance and is flexible to meet the needs of the agency.
- Follow all Medicaid and HIPAA regulations and requirements.
- Abide by all regulations, policies, procedures and standards.
- Performs other duties as assigned.
Position Requirements & Competencies:
- Graduation from an accredited school of nursing and current LPN licensure in the state of practice
- New Nurses welcome, will train!
- Ability to make home visits and provide nursing care in the home environment
- No weight restrictions
- Home Care experience preferred but not mandatory
- Must have reliable transportation, current driver's license and appropriate automobile insurance; or ability to take public transportation
- Ability to complete necessary documentation appropriately, legibly, and timely
- Interpersonal, verbal, and written communication skills necessary to complete job responsibilities
Skills Required
