What you will do:

• Ready to manage a panel of members to maintain their health holistically.
• Experienced in comprehensive care management, chronic disease management, urgent home-based and community-based primary health visits, preventative care and wellness.
• Experienced and comfortable with virtual patient engagement, patient centered action and/or care plans, resource management.
• Strong communication and relationship skills with demonstrated success partnering with providers and other team members to support wellness and prevention.
• Adaptable, organized, efficient, independent self-starter and problem-solver, a leader, a strategic thinker, and a mentor, who is excited about the big picture of whole community health.

Degree Requirements and Certifications:

• Board-Certified Nurse Practitioner or Physician Assistant with an unrestricted license to practice in Ohio
• AANC or AANP certification for Nurse Practitioner
• Active DEA Controlled Substance Registration or eligible for application
