Qualifications and Requirements:
- Responsible for the complete referral process, including identifying home health needs, verifying benefits, and coordinating care.
- Cultivate new and existing relationships resulting in company growth.
- Initiate and expand referral network.
- Comply with all aspects of the company's compliance and HIPPA regulations.
- One year of home health, marketing, or case management experience or a combination of each.
- Must possess a valid driver's license.

$85,000 - $95,000
Skills Required
- Manage complete referral process including identifying home health needs, verifying benefits, and coordinating care.
- Cultivate new and existing relationships to drive company growth.
- Initiate and expand referral network.
- Comply with company compliance and HIPAA regulations.
- One year of home health, marketing, or case management experience (or combination).
- Valid driver's license.
