Coordinate daily schedules for home health aides, field new patient/client inquiries by phone, monitor and log caregiver and client activity, follow up on assignments and services, ensure optimal use of CICOA/WAIVER/VA/prior authorization hours, and participate in an on-call rotation supporting aides and residents.

Coordinate hospital-to-home health referrals and transitions, build relationships with hospital case managers and discharge planners, manage and follow up on referrals urgently, educate patients and families, conduct onsite hospital visits, collaborate with intake and clinical operations, and optimize referral conversion and patient experience using referral/EMR platforms.
