Provide case management for clients with substance use and mental health needs: assess, develop and implement care plans, coordinate medical/housing/employment resources, enter HMIS data, oversee medication management, assist with benefits and discharge planning, maintain documentation, and participate in treatment team and quality improvement activities.

Provide case management to children and families involved in child welfare: assess needs, develop and monitor case plans, coordinate services, document in CCWIS/Stabilify, attend court, provide safety management, support permanency and family connections, and collaborate with schools and community partners.
