Manage a caseload of low-income veterans using Housing First principles: assess needs, create and monitor individualized plans, coordinate referrals, provide advocacy, conduct outreach and home visits, mentor junior advocates, document services, and connect veterans to housing, employment, and community supports.

Provide primary intensive outpatient nursing care for women with substance use or co-occurring disorders: assess and document medical histories, administer and track medications, assist physician with exams and treatments, perform vitals and routine tests, contribute to treatment planning and documentation, support clients' physical and psychosocial needs, and complete required reports.

The Case Manager provides intensive support and resources to individuals experiencing homelessness, facilitating care plans, client advocacy, and coordination with various service providers.
