Collects, tracks, and enters client and program data; conducts admission, discharge, and follow-up assessments; maintains statistical databases; analyzes and reports outcomes; supports funder compliance, performance improvement plans, electronic health record implementation, documentation audits, and quality assurance activities. Requires interaction with substance use and mental health clients, evaluators, supervisors, and agency teams.

Provides peer-based behavioral health and substance use recovery support for individuals served by a mobile mental health unit. Responsibilities include crisis stabilization assistance, peer mentoring, recovery education, resource navigation, outreach, group facilitation, care coordination, service planning, documentation, and collaboration with clinical teams. The role requires lived recovery experience, professional boundaries, community engagement, local travel, and support during varied hours.

Provides person-centered case management and care coordination for individuals experiencing behavioral health concerns or crises. Conducts assessments, supports crisis stabilization, develops service plans, coordinates referrals and warm handoffs, provides brief interventions and education, performs community outreach, documents services, and collaborates with interdisciplinary teams. Operates the mobile mental health unit and transports clients as authorized. Regular travel and variable hours, including evenings, weekends, and holidays, may be required.
