Provides case management, care coordination, continuous assessment, treatment planning, and psychiatric service coordination for individuals with serious mental illness. Coordinates care across mental and physical health systems, manages court-ordered treatment processes, maintains accurate documentation, and collaborates with clinical teams and legal counsel.

Provides comprehensive primary care to children and young adults, including physical examinations, preventive visits, vaccinations, screenings, diagnosis, treatment, medication management, developmental evaluations, and referrals. Educates families, coordinates with healthcare providers, performs or interprets diagnostic testing, and supports culturally sensitive care in a collaborative community health setting.

Educates and enrolls eligible individuals and families in SNAP, conducts community outreach, screens clients for eligibility, assists with applications, develops partnerships, documents activities, tracks enrollment metrics, and connects low-income households with food and community resources.

Coordinates and grows statewide leadership development programs, including participant recruitment, alumni engagement, events, workshops, curriculum improvement, community partnerships, sponsorships, fundraising support, reporting, and outreach. The role requires regular travel throughout Arizona, public representation, cross-functional collaboration, and frequent evening or weekend work.

Provides psychiatric rehabilitation, vocational support, and community integration services to adults with serious mental illness. Conducts assessments, develops individualized interventions, builds independent living and employment skills, coordinates referrals, and collaborates with interdisciplinary teams, families, employers, and community organizations. Maintains service documentation, monitors outcomes, supports quality improvement, and provides culturally responsive, trauma-informed, recovery-oriented care in clinical and community settings.

Answers inbound calls, assists patients with appointments and service inquiries, processes medication prior authorizations and outbound referrals, and manages incoming e-faxes. Scans documents into patient electronic health records and performs general clerical and administrative tasks. The role requires strong communication, organization, cultural sensitivity, reliable transportation, required health certifications and screenings, and a valid Fingerprint Clearance Card.

Provides front desk administration for an integrated primary care and behavioral health organization. Responsibilities include data collection, data reporting, general administrative support, and assisting staff under the direction of the Director or Office Coordinator. The role requires bilingual Spanish communication, culturally sensitive service, basic Windows 365 or electronic health records experience, reliable transportation, healthcare certifications, fingerprint clearance, and required health screenings and vaccinations.

Supervise school-based behavioral health staff and provide clinical oversight for youth mental health services. Guide treatment for dual diagnoses, trauma, and behavioral challenges; support family systems and play-based therapies; monitor caseloads and visit volumes; deliver staff training; and build collaborative relationships with school administrators and staff. The role also maintains clinical and administrative documentation, supports conflict resolution, and ensures culturally sensitive, trauma-informed care.
