Coordinate and manage completion of MDS assessments, ensure regulatory compliance, develop care plans with the interdisciplinary team, audit documentation for quality, optimize PDPM reimbursement, provide staff education, manage data reporting, and support resident care across the skilled nursing facility.

Provide direct nursing care in a skilled nursing facility: administer medications, monitor vitals, implement care plans, perform basic wound care, document thoroughly, respond to emergencies, educate residents/families, and collaborate with the interdisciplinary team to ensure resident safety and comfort.

Coordinate and plan individualized discharges for SNF residents by assessing needs, arranging post-discharge services (home health, equipment, follow-ups), documenting plans, communicating with families and providers, advocating for residents, providing education, and performing follow-up to ensure smooth care transitions and regulatory compliance.

Support day-to-day operations by coordinating projects, improving processes, managing data and vendors, providing administrative support, ensuring compliance and assisting with payments/collections. Produce reports and dashboards, track metrics, support audits, and collaborate across departments to optimize workflows and operational effectiveness.
