Support social services in a long-term care and short-term rehab nursing center by admitting residents, completing psychosocial assessments, care and discharge plans, coordinating with Social Services Manager, and ensuring positive patient experiences during treatment. Work is Monday–Friday day shift.

Lead orientation, clinical education, and competency evaluations for nursing staff in a skilled nursing facility. Mentor new nurses, maintain training records, ensure regulatory compliance, and support quality improvement and retention initiatives.

Develop, implement, and coordinate the Resident Assessment Instrument (RAI)/MDS process for residents. Ensure compliance with professional practice standards, physician orders, company policies, and local, state, and federal regulations. Support documentation, interactions with payers (Medicare/Medicaid), and quality of care in a long-term care setting.
