Provides direct nursing care to patients and residents in skilled nursing, sub-acute, and long-term care settings. Responsibilities include administering medications, changing wound dressings, providing treatments, performing physical examinations, and conducting diagnostic tests. The role requires completion of an approved practical or vocational nursing program and an active LPN license in the state of practice. Baylor staff work two 16-hour weekend shifts and receive additional wages.

Directs and evaluates nursing care for residents in a long-term care and skilled nursing facility. Manages clinical department operations, quality of care, regulatory compliance, budgeting, and clinical staff leadership. Serves as backup to the Nursing Home Administrator. Requires an active RN license, nursing degree, and at least one year of nursing leadership experience.

Coordinates the Resident Assessment Instrument and Minimum Data Set processes in a long-term care center. Ensures assessments are developed, implemented, and maintained in compliance with professional standards, physician orders, company policies, and applicable regulations. Requires an active state RN license and knowledge of Medicare, Medicaid, OBRA, long-term care payer systems, RAI, and MDS processes. Prior MDS or long-term care nursing experience is preferred.

Manage nursing care for resident patients, maintain and restore health, serve as the primary contact for residents and families, and address or prevent family concerns. The role requires an active RN or LPN license, an associate degree or higher in nursing or a practical nursing diploma, and at least one year of nursing experience. Sub-acute or long-term care experience is preferred.
