Provide direct client care and medication administration in a behavioral health urgent care setting. Monitor and document patients' physical and mental condition, report to the RN and physician, confirm medication deliveries, write client summaries, and assist with shift nursing assignments to maintain safety and quality care.

Provide community-based case management for transitional-age youth: conduct assessments, deliver services in-office and in homes, coordinate treatment team meetings, document services in the electronic medical record, monitor progress toward treatment goals, and use a personal vehicle for client visits and occasional client transport.

Provide direct support to adults in a 16-bed crisis residential unit: identify housing and community resources, coordinate outreach, assist with admissions consent, facilitate groups and resident meetings, and transport clients to appointments to link them to services and supports.

Manage the Quality Assurance and Medical Records program for a TAY Crisis Residential Treatment facility. Audit clinical documentation, reconcile charts with billing, ensure Medi-Cal, county, and internal policy compliance, track and route chart deficiencies for correction, oversee CQI data, and ensure discharged charts are completed and closed.

Provide direct psychiatric nursing care in a 16-bed crisis residential treatment setting. Work in community, clinic, home, and occasional telehealth sessions across Santa Clara County; transport clients and perform home-to-home visits. Collaborate with a multidisciplinary team focused on prevention and early intervention for individuals at risk of early psychosis and support families.

Supervise, train, evaluate, and discipline clinical staff; provide ongoing in-service training and ensure regulatory-compliant orientation and continuing education; maintain and revise program policies and procedures; coordinate with county agencies and community resources to support adult inpatient recovery, rehabilitation, and related services.

Develops, coordinates, and delivers clinical training across the Southern Region. Oversees new employee orientation, required in-service and specialized clinical trainings, maintains training records, delivers sessions, partners with counties and universities, secures trainers, and develops joint training initiatives supporting treatment of seriously emotionally disturbed clients.

Provide peer-based outreach, engagement, advocacy, and direct support using lived experience to help Transitional Age Youth stabilize, recover, and transition back to the community.
