Performs Medicare Annual Wellness Visits collaborating with PCP and care team: outreaches eligible patients, completes AWV protocols and preventive services, documents in the electronic record, addresses quality gaps, assists with scheduling and other clinical tasks, and provides patient advocacy and education using nursing assessment and triage skills.

Manage vendor invoices and payments, apply credits and sales tax, reconcile accounts monthly, analyze expenses and account activity, resolve invoice discrepancies, and support monthly financial reporting.
