Conducts Medicare Annual Wellness Visits, identifies preventive care and quality gaps, follows clinical protocols, documents care in the electronic record, coordinates follow-up, educates patients, connects them with community resources, and advocates for designated patient populations. May also support prior authorizations, prescription refills, patient messages, and other clinical tasks while collaborating with primary care teams.
