Directs nursing care operations in a skilled nursing facility, supervising nursing and support staff, assigning care based on resident needs and staff competencies, overseeing medications and treatments, monitoring resident conditions, coordinating with physicians and multidisciplinary teams, maintaining compliant documentation, and supporting long-term, sub-acute, memory care, and specialized services.

The RN/LPN Unit Manager plans, supervises, and evaluates nursing care in a skilled nursing, sub-acute, or memory care unit. Responsibilities include assessing residents, developing care plans, administering medications and treatments, monitoring conditions, supervising staff, coordinating multidisciplinary care, educating families, maintaining compliant documentation, and supporting case management and specialized services.

Provide and coordinate resident-centered long-term, sub-acute, and memory care by completing MDS and PPS assessments within regulatory timeframes, transmitting and validating MDS to CMS, driving interdisciplinary care plans, maintaining accurate documentation and schedules, participating in case management and Medicare skilled meetings, and collaborating with clinical teams to ensure regulatory compliance and continuity of care.
