Manage and oversee Individual Support Plans (ISPs) for people with intellectual/developmental disabilities and behavioral health needs. Coordinate IDT meetings, conduct announced and unannounced residential visits, document communications, respond to incidents, train staff, perform QA assessments, support health record accuracy, and provide coverage/on-call duties as needed.

Provide nursing oversight and community-based care for people with intellectual/developmental disabilities and behavioral health needs as part of an ACTT.
