Provides community-based crisis intervention, prevention, case management, referrals, care coordination, and training for individuals with intellectual and developmental disabilities and behavioral health needs. Develops and monitors cross-systems crisis plans, consults on clinical care, supports families and stakeholders, and participates in emergency and respite services. Requires travel, crisis management experience, direct support experience, and applicable professional certifications.

Conducts audits of service settings, records, personnel, and service delivery to ensure regulatory and policy compliance. Performs follow-up audits, analyzes quality data, supports QAPI meetings and improvement plans, provides quality-related training, assists with investigations and policy development, and coordinates meetings and special projects across multiple units.

Provides blended case management for individuals with intellectual and developmental disabilities, mental health needs, and substance use challenges. Develops service and crisis plans, connects clients with community resources and benefits, advocates across social service systems, maintains documentation, monitors client wellbeing, coordinates with treatment providers, and provides rotating 24/7 crisis support. Deaf Services employees must be fluent in American Sign Language.
