Coordinates employment and community-based services for individuals with intellectual and developmental disabilities and behavioral health or substance use needs. Responsibilities include intake assessments, person-centered planning, service authorization and monitoring, employment planning, job coaching, documentation, stakeholder communication, community outreach, and possible supervision of job coaches or direct support professionals. Requires a high school diploma, reliable transportation, a valid driver’s license, and state-mandated employment certification.

Provides prescribed medical treatment, medication support, personal care, vital-sign monitoring, and advocacy for individuals with intellectual and developmental disabilities and behavioral health needs. Assists registered nurses and interdisciplinary teams, observes and reports reactions, maintains medication processes, and responds calmly during crises. The role requires CPR capability, physical lifting, a valid LPN license, a driver’s license, and background, registry, drug, and credentialing checks.

Provides broad HR support across a six-state organization, including employee relations, investigations, recruitment, onboarding, compliance, benefits, performance management, reporting, and acquisition integration. Partners with HR Business Partners, advises employees and managers, maintains confidential HRIS records, supports audits and policy updates, and assists with change management. The role requires sound judgment, strong communication, confidentiality, organizational skills, and frequent travel, including overnight stays.
