Provide RN case management for Medicaid CAP participants: perform assessments, develop and update person-centered care plans, coordinate and monitor services, conduct home visits and phone follow-ups, ensure Medicaid compliance and documentation, provide family counseling and training, and complete required state trainings.

Provide community-based nursing care and oversight for individuals with intellectual/developmental disabilities and behavioral health needs. Participate on interdisciplinary ACTT team, follow crisis and person-centered plans, monitor medication side effects, teach coping skills, and perform required documentation and nursing interventions.

Manage the full revenue cycle and accounts receivable tasks including billing (Medicaid and other payers), cash posting and reconciliation, claims research and appeals, collections, account setup, reporting, and maintaining accurate AR records and documentation.
