Manage and oversee Individual Support Plans (ISPs) for people with intellectual/developmental disabilities and behavioral health needs. Coordinate IDT meetings, conduct announced and unannounced residential visits, document communications, respond to incidents, train staff, perform QA assessments, support health record accuracy, and provide coverage/on-call duties as needed.

The Contract RN provides nursing care to individuals with behavioral health and substance use challenges, participating in interdisciplinary teams and crisis response while ensuring quality care and compliance with guidelines.

Provide RN case management for Medicaid CAP participants: perform assessments, develop and update person-centered care plans, coordinate and monitor services, conduct home visits and phone follow-ups, ensure Medicaid compliance and documentation, provide family counseling and training, and complete required state trainings.
